The letter must identify the identity of the requestor, i.e. the person who is the subject of the data, with his/her surname, first name and address, the subject of the request, the information that the person expects to receive in order to guide potential searches and, optionally, the purpose of the request. MUA MULTI FAMILY OFFICE may request proof of identity when examining the request in order to guarantee the applicant's identity.
